A free lookup on the USPTO database tells you whether someone has registered your exact words. It does not tell you about the phonetically similar mark in a neighbouring class, the state registration nobody federalised, or the company three states over that has been using the name in commerce since 2014 and therefore has common-law rights that beat your application.
A comprehensive search is the cheapest insurance in the whole process. The alternative is finding out after you have printed packaging, bought the domain, paid for a brand identity and started running ads.

No, and anyone who tells you otherwise is overselling. Examination involves judgement, and rights can exist that no search surfaces. What a clean report does is remove the foreseeable risks and give you a documented basis for going ahead.
Yes. Design marks are searched on their design elements as well as any wording, which is why logo clearance is a separate exercise from word-mark clearance.
